Telmisartan is an angiotensin II receptor blocker (ARB) that prevents the vasoconstriction and aldosterone-secreting effects of angiotensin II by selectively blocking AT1 receptors. It does not inhibit ACE (kininase II), nor does it interact with other hormone receptors or ion channels important in cardiovascular regulation.
Hydrochlorothiazide is a thiazide diuretic that increases sodium and chloride excretion, leading to plasma volume reduction. This results in an increase in plasma renin activity and aldosterone secretion, which may cause potassium loss. When combined with an ARB, the potassium loss is counteracted.

Safety and effectiveness in children have not been established.
Older patients should start with the lowest effective dose due to possible decreased kidney, liver, or heart function.
